Skilled nursing & rehabilitative care Community Health Center also has Certified Medicare Skilled beds. All patients on the SNF unit are able to receive all the services and benefits available to Community Health Center residents. Professional therapists work with residents to promote physical, mental, and emotional wellness. Each individual is assessed to find the best treatment for their individual case.

Respite & restorative services CHC offers short-term care programs for caregiver relief (Respite) as well as for rehabilitation and restorative service. Each program is designed to meet each individual requests and desires.

Independent retirement apartments Community Health Center has eight independent living retirement apartments. One and two bedroom units are available. Independent living residents are welcome to participate in any activities offered at CHC as well as dining in our Garden View Café.
